Northwest Regional Human Rights Authority Minutes

February 10, 2026

The regular stated meeting of the Northwest Regional Human Rights Authority (HRA) was called to order at 4:06pm February 10, 2026 , by Chari Dr. Erin Wade at the  Rockford Regional Office (RRO) -4302 North Main Street, Conference Room, Rockford, IL 61103.

Roll Call

Members Present: Colleen Magee, Collen Parks, Dr. Erin Wade and Zoe Wedel

Members Approved to Join Virtually: Sang Capone

Members Absent: Christine Dungey

Staff Present: Nicole Weathersby, DRM

Guests: N/A

Dr. Erin Wade stated a quorum was/was not present.

Dr. Erin Wade read the confidentiality statement reminding all present that personally identifying information should not be presented in public meetings to protect the privacy of those persons with disabilities on whose behalf the HRA conducts investigations.

Dr. Erin Wade  asked for any changes to the agenda, there were no changes. .

Introductions are made for any visiting guests.

Minutes:

The January 6, 2026, open and closed meeting minutes were reviewed. Zoe Wedel  motioned to approve the minutes; Colleen Parks seconded, and the motion carried. The corrected  August 19, 2205, general minutes were reviewed. Zoe Wedel motioned to approve the minutes and repost, Colleen Parks seconded, and the motion carried. The corrected  September 23, 2205, general minutes were reviewed. Zoe Wedel motioned to approve the minutes and repost, Colleen Parks seconded, and the motion carried.

B.      New Complaints

1.      26-080-9019 ( Behavioral Health Unit) – Complaint Summary: The complaint alleges that the facility imposed “ emergency medication injection”.  Allegedly,  the individual  was not given the right to refuse medication and treatment. The complainant alleges being mishandled by security. Allegedly, the individual stated not being provided with adequate and humane care and services in the least restrictive environment. Colleen Parks made a motion not to accept the complaint , due to the complainant not wanting to move forward. The motion was properly seconded by Zoe Wedel. Motion carried.

 

2.      26-080-9020 ( Behavioral Health Unit) – Complaint Summary: The complaint alleges that the facility mislead what the voluntary application process was, including what the patients’ bill of rights were. Colleen Parks made a motion not to accept the complaint, due to the complainant not wanting to move forward. The motion was properly seconded by Zoe Wedel. Motion carried.

 

3.      26-080-9021( Behavioral Health Unit) – Complaint Summary: The complaint alleges being a voluntary recipient in the  facility imposed “ emergency medication injection”, unnecessarily. Zoe Wedel motion to table the complaint, pending receipt of a signed consent. The motion was properly seconded by Colleen Magee.  Motion carried.

 

4.      26-080-9022( Behavioral Health Unit) – Complaint Summary: The complaint alleges the facility did not complete a requested “assault exam”. Allegedly, the individual was deprived of their guaranteed rights. The complainant alleges that the facility refused to contact law enforcement. Allegedly, the individual did not receive adequate, humane  care and services in the least restrictive environment. Zoe Wedel made a motion to accept , open and pursue the complaint , pending discharge. The motion was seconded by Colleen Maggee. Motion carried.

 

Closed Session:

 

Zoe Wedel made amotion to enter closed session at 4:21pm, pursuant to Section 120/2 (4) and (10) of the Open Meetings Act (5 ILCS 120) to discuss preliminary case information and Section 21 of the Guardianship and Advocacy Act and Section 110/5 of the Mental Health and Developmental Disabilities Confidentiality Act (740 ILCS 110) to protect the confidentiality of service recipients.  The motion was seconded by Colleen Magee and carried as follows: Sang Capone – Yes, Colleen Magee -Yes, Colleen Parks – Yes, Dr. Erin Wade – yes, and Zoe Wedel – Yes. The purpose of closed session was to discuss preliminary and confidentiality parts of various cases.

The HRA returned to Open Session at 4:49pm having taken no action in closed session. Executive minutes were recorded.  In closed session, the HRA reviewed preliminary reports concerning cases # 25-080-9002 and confidential case information concerning case # 25-080-9017,  #25-080-9025, #25-080-9027, #26-080-9007 and #26-080-9022.

  1. Draft Reports

#25-080-9022– Northwestern Medicine Kishwaukee Hospital – Colleen Parks motioned to approve the draft report. The motion was properly seconded by Colleen Magee. Motion carried.

  1. Case Closures

#25-080-9025– UWHealth/SwedishAmerican Hospital – Zoe Wedel motion to deny case closure, due to never receiving an updated signed consent.  The motion was properly seconded by Colleen Magee. Motion carried.

#25-080-9027– Winnebago County Justice Center - Zoe Wedel motion to deny case closure, due to never receiving an  updated signed consent.  The motion was properly seconded by Colleen Magee. Motion carried.

#26-080-9007 UWHealth/SwedishAmerican Hospital – Zoe Wedel motion to deny case closure, due to never receiving an updated signed consent.  The motion was properly seconded by Colleen Magee. Motion carried.
